超聲引導(dǎo)下不同穿刺速度對甲狀腺結(jié)節(jié)細胞學(xué)診斷率的影響
[Abstract]Objective: To explore and analyze the influence of diferent puncture speeds under ultrasound guidance on the cytological diagnosisrate of thyroid nodules.Method:A total of 8O patientswith thyroid nodules who undergoing ultrasound-guided puncture in Pingxiang People's Hospital from January 2O22 and December 2024 were enrolled,according to the simple and random grouping method, they were divided into the routine group [normal-speed puncture (operation time >3 s)] and the high-frequency group [high-frequency and rapid puncture (operation time ?3 s)], 40 cases in each group. The sampling satisfaction rate,cytological diagnosis rate and occurrence of puncture complications were compared between two groups.Result: The sampling satisfaction rate in the high-frequency group was 95.00% , which was significantly higher than 80.00% in the routine group, the difference was statistically significant ( P <0.05).The accuracy rate of cytological diagnosis in the routine group was (204號 84.38% ,and sensitivity was 88.00% , specificity was 71.43% ,and Kappa value was O.565,showing good consistency; the accuracy rate of cytological diagnosis in the high-frequency group was 92.11% ,and sensitivity was 93.75% , specificity was 83.33 % ,and Kappa value was O.723,showing high consistency; there were no significant differences in the accuracy,sensitivity and specificityof two groups' puncture speeds (P>O.O5). There was no significant diference in complications such as puncture site pain,recurrent laryngeal nerve injury,local hematoma and infection between the routine group and the high-frequency group (P>0.05 ). Conclusion: High-frequency puncture under ultrasound guidance can improve the rate of cytologicaldiagnosis and the sampling satisfactionrate of thyroid nodules,reduce non-diagnostic results,and thus avoid unnecessry repeated puncture or surgery.
