【Abstract】 ObjectiveTo explore the ultrasonographic characteristicsand value of thyroid micropapillary carcinoma (PTMC) in the background of Hashimoto thyroiditis (HT).MethodsNinety-eight patients inour hospital fromJuly2021toDecember 2O23wereconfirmedasHTbysurgical pathologyinourhospital,and79patients (82 nodules)with the largest diameter of thyroid nodule ?1 cm were selected as the subjects of this study.All patients underwent preoperative ultrasound examination.Benign and malignant thyroid nodules in HT patients were analyzed, ultrasonic characteristicsofdiferentthyroidnodulesinHTpatientswerecompared,andanterogradeandanterograde nodular diameters and anterograde diameters in PTMCand benign micro-nodules in HT patients were compared.The "gold standard" was the pathological examination results.The diagnostic value of ultrasound in PTMC in HT patients was analyzed,and the consistency with the "gold standard" was calculated.ResultsFifty-four malignant nodules and 28 benignnodules were confirmed pathologicallyin79 patients with HT(82 nodules).Inthe contextofHT,the detectionrates of microcalcification,low blood supplydegreeand marginal bloodsupply patern in PTMC patients were higher than those of benign micro-nodules,with statistical differences ( P <0.05). There were no significant diffrences ininternalecho,nternal structure,shape,halo,boundaryandaspectratio (P>O.O5).InthecontextofHT, thediameter of thyroid nodules in PTMC patients was larger than that of benignminor nodules,with statistical difference ( P<0.05 ). The sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 93% , 54% ,79 % , 79% and 79% respectively in the diagnosis of PTMC in HT background.Consistency test showed thatthe consistency between ultrasound examination and pathological examination in the diagnosis of PTMC inthe context of HT was reasonable (Kappa value=0.500,P<0.001).ConclusionUltrasound in the apicationof PTMC underthe background of HTcaneffectivelydefineultrasonic image features,andthediagnosticconsistencyis reasonable,whichcan provide efectivereference for clinical diagnosisandtreatment,and isworthyof popularization and application.
