Neuroendoscopic-assted Treatment of Basal Ganglia Intracerebral Hemorrhage by Different Access outes/MO Qiulian, LI Chunhui. //Medical Innovation of China, 2025, 22(11): 134-138

[Abstract]Objective: To discuss the efficacy of neuroendoscope-assisted transsylvian insula approach and transtemporal cortex approach in the treatment of basal ganglia intracerebralhemorrhage.Method:A totalof 120 patients with confirmed spontaneous basal ganglia intracerebral hemorrhage in the First Hospital of Hunan University of Traditional Chinese Medicine from January 2022 to June 2024 were selected for retrospective study, and were divided into 60cases in the transsylvian insula group and 60cases in the cortexgroup according to the surgicalaccessroutes.Both groupsunderwent neuroendoscopic surgicaltreatment, withthe transsylvian insula group taking atranssylvian insula approach and the cortex group takingatranstemporal cortex approach.The degree of cerebral hematoma clearance, Glasgow prognostic score before and after surgery,and the occurrence of postoperative complications were compared between the two groups.Result:The residual hematoma volume of the transsylvian insula group after surgery was lowerthanthat ofthe cortex group,hematomaclearancerate was higherthanthat of cortex group, the differences were statistically significant ( P <0.05).The incidence of postoperative complications in the transsylvianinsula group was lowerthanthat in the cortex group,thedifference was statisticall significant ( P <0.05).After a 6-month follow-up, according to the Glasgow prognostic score evaluation, 42 cases (70.00 % ) in the transsylvian insula group had a good prognosis; there were 20 cases in the cortex group, accounting for 33.33% , the difference was statistically significant ( P <0.05). Conclusion: The neuroendoscopic surgery using the transsylvian insula approach has shown significant results in the treatment of basal ganglia cerebralhemorrhage,compared with transtemporal cortex approach surgery,this surgery is more efective inclearing hematoma, and the surgical operation is simpler, effectively improving treatment efficiency.
