Comparison of the effect of peripherally inserted central catheters between the left upper limb and the right upper limb in adults：a Meta-analysis

Abstract Objective：To systematically evaluate the catheterization effect of PICC in left and right upper limbs of adults.Methods： Studies on the comparative effect of PICC insertion in the left and right upper limbs of adults were searched from CNKI，WanFang Database，CBM，PubMed，Web of Science，EMbase and the Cochrane Library，and the search period was from the establishment of the database to May 7，2023. Literature screening，evaluation and data extraction were conducted independently by two researchers，and Meta-analysis was performed using RevMan 5.3 software.Results： A total of 26 articles were included，involving 5 489 patients with left upper limb PICC catheterization and 7 425 patients with right upper limb PICC catheterization. The results of Meta-analysis showed that：compared with the right upper limb，the risk of secondary ectopia，catheter rupture，and blood oozing occurred in PICC catheterization in the left upper limb was smaller，which increased the comfort of patients. However，catheter catheterization in the left upper limb was more difficult than that in the right upper limb，and the risk of phlebitis in the right upper limb is low，with statistical significance （P< 0.05）.Conclusion： Existing evidence shows that PICC catheterization of the left and right upper limbs in adults has advantages and disadvantages，clinical nurses should abandon the concept of "right upper limb is the first choice"，and consider the complications during catheter retention and the patient′s wishes before catheterization，rationally select the upper limb for catheterization.
