【Abstract】 ObjectiveTo explore the application value of real-time shear wave elastography (SWE)technology inoptimizing Chinese thyroid imaging reporting and data system(C -TIRADS)clasification of thyroid nodules. Methods Atotal of 122 patients (146 nodules)with thyroid nodules who visited the Cardiovascular and Cerebrovascular Hospitalof Ningxia Medical University from January202 to December 2023 werecollcted.Before surgery or puncture,routine ultrasound C-TIRADS clasificationand SWE examination optimizationclassification wereperformed.The quantitative parameter Emax value of SWE was selectedas 57.OO kPaand pathological results wereusedas the "goldstandard"tocompare theeficacy differences between thetwogroups in diagnosing benign andmalignant thyroid nodules.ResultsRoutineultrasound diagnosisof 83 malignant thyroid nodules and 63 benign thyroid nodules,witha sensitivityof 82.9% ,specificity of 76.6% ,positivepredictive value of 81.9%,negative predictive value of 77.8% ,and accuracy of 80.1% .After optimizing the classification of C-TIRADS using SWE technology,75malignant nodulesand71benign nodules were diagnosed,with asensitivityof93.4%,specificityof 94.3% ,positivepredictive value of 94.7% ,negative predictive value of 93.0% ,and accuracy of 93.8% After optimization of SWE technology,the specificity ( χ2 =17.72, P =0.001),positivepredictive value( χ∠ =6.04, P =0.014), negative predictive value ( χ2 =6.32, P=0.012 ),and accuracy ( χ2 =12.10, P <0.001) of C-TIRADS classification for the diagnosis of benign and ma-lignantthyroid nodules were significantly higher than those of conventional ultrasound CTIRADS classification. The difference between the two was statistically significant ( P <0.05), while the sensitivity χ2= 0.54, P=0.463 )showed no statistically significant difference P>0.05 )compared to conventional ultrasound C - TIRADS classfication.ConclusionSWE technology can significantly optimize and correct the clasification of thyroid nodules using conventional ultra-sound C-TIRADS.

隨著我國全民健康體檢的逐步普及,甲狀腺結(jié)節(jié)的檢出率逐年呈上升趨勢,在健康人群中高達50%~60% ,其中甲狀腺癌的患病率為 5%~15%[1] 號近20年以來中國甲狀腺癌發(fā)病率逐年增高且女性明顯高于男性[2],已躍居女性惡性腫瘤的第4位[3],成為嚴重威脅人們身心健康的高發(fā)惡性腫瘤之一。(剩余6567字)
