【Abstract】ObjectiveTo establish andvalidate a magnetic resonance imaging (MRI) radiomics model to predict extramural venous invasion (EMVI) inrectalcancer.MethodsA total142 patients with rectal cancer were included inthisstudy(85wereEMVIpositiveand57wereEMVInegative),99 patients wereonthetraining set,and 43 patients were on the testing set.T 2 -weighted imaging )and diffusion-weighted imaging (DWI) imageswere acquired from the patients before surgery.Atotal 398radiomics features wereextracted from the DWI and TWI images,respectively,and 796 in total. The maximum relevance minimum redundancy (mRMR) and theleastabsolute shrinkageand selection operator(LASSO) were used to select the final radiomics features.The radscore model was established toevaluate the EMVIstatus rectal cancer.The predictive performance the model wasevaluatedusing the?receiver operating characteristic curve (ROC),accuracy,sensitivity,specificity,positive predictive value (PPV),and negative predictivevalue (NPV).ResultsA total 23 radiomics features were selected toconstruct theRad-score model.The model performedwellinboth the training setand testing set,withan areaunder the curve(AUC) 0.91( 95% confidence interval [95%Cl]:O.88,O.95) in the training set and 0.72 (95%CI: 0.55,0.89)inthevalidationset.Theaccuracy,sensitivity,specificity,PPV,andNPVinthe trainingset were 0.86,0.83,0.88,0.83,and0.88,respectively.Inthe testingset,theresults were0.69,0.59,0.76,0.63,and 0.73.ConclusionThe rad-score model demonstrates good diagnostic performance inpredicting the EMVI rectal cancer before surgery.Therefore,it can be used as a key tool for predicting EMVI in rectal cancer.
