Risk factors of venous thromboembolism during pregnancy and puerperium：a Meta-analysis

AbstractObjective：To identify the risk factors of venous thromboembolism during pregnancy and puerperium by Meta-analysis，so as to clarify its risk factors.Methods：Studies on the risk factors of venous thromboembolism during pregnant and puerperal were searched from CNKI，WanFang Database，VIP，CBM，PubMed，Web of Science and EMbase.The retrieval time was from the establishment of the database to March 1，2022.RevMan 5.3 software were used for Meta-analysis.Results：A total of 24 studies were included，involving 11 risk factors.Meta-analysis results showed that age（≥35 years）［OR=1.94，95%CI（1.59，2.37），P<0.001］，history of thrombosis［OR=2.23，95%CI（1.49，3.34），P<0.001］，cesarean section［OR=2.17，95%CI（1.95，2.42），P<0.001］，gestational diabetes［OR=1.83，95%CI（1.25，2.66），P=0.002］，gestational hypertension［OR=3.21，95%CI（1.93，5.34），P<0.001］，eclampsia or preeclampsia［OR=2.48，95%CI（1.80，3.41），P<0.001］，sedentary in bed［OR=2.73，95%CI（2.08，3.59），P<0.001］，high D-dimer［OR=1.63，95%CI（1.12，2.38），P=0.01］，twin or multiple pregnancies［OR=2.64，95%CI（2.19，3.18），P<0.001］，history of contraceptives before pregnancy［OR=2.24，95%CI（1.38，3.64），P=0.001］ and body mass index（≥24 kg/m2） ［OR=2.10，95%CI（1.37，3.24），P<0.001］were the risk factors of venous thromboembolism during pregnancy and puerperium.Conclusions：Current evidence shows that age（≥35years），history of thrombosis，cesarean section，gestational diabetes，gestational hypertension，eclampsia or preeclampsia，sedentary in bed，high D-dimer，twin or multiple pregnancies，history of contraceptives before pregnancy and body mass index（≥24 kg/m2） were the risk factors of venous thromboembolism during pregnancy and puerperium.Clinical evaluation and management should be strengthened.However，due to the medium quality of the studies included，more high-quality studies are needed to explore the risk factors.
