【Abstract】Objective To investigate the value of prostate volume，bladder detrusor muscle thickness， and bladder detrusor muscle two-dimensional shear wave measurements in predicting bladder detrusor underactivity （DU）. MethodsThe studygroup included 174patients with benign prostatic hyperplasia who underwent urodynamic testing from June 2O16 to December 2O23 inour hospital.The prostate volume and bladderdetrusor muscle thickness were measured using two-dimensional ultrasound，and the shear wave elasticity of the bladderdetrusor muscle was measured using two-dimensional ultrasound shear wave elastography.ResultsThe sensitivityand specificityof prostate volume （PV） ? 46.68 ml for predicting DU were 74.7% ， 100% ， and area under ROC curve （AUC） was 0.798， respectively. Detrusor wall thickness （DWT）≤1.63 mm predicted that the DU sensitivity was 83.8% ， the specificity was 86.1% ，and the AUC was O.881.The DU sensitivity of bladder anterior wall 2D-SWE≥10.15kPΛ a，posterior wall 2DSWE?7.69 kPa，left lateral wall 2D-SWE ? 9.2 kPa， right lateral wall 2D-SWE≥7.3 kPa were 88.9%， 66.7%， 82.8%， （20 62.6% . The specificity were 94.4%， 47.2% ， 36.1% ，and 58.5% . The AUC values were 0.936， 0.565， 0.512，and 0.610. ConclusionTwo-dimensional ultrasound can be used to measure the prostate volume and detrusor muscle thickness， andtwo-dimensional ultrasonic shear wave elastic imaging can directly observethe change of detrusor muscle hardness.The increase of shear wave hardness ofeach bladder wallcan predictthe decrease of bladder function.The two-dimensionalshear wave elastic imaging of the front wall of the bladdercan predictthe detrusorfunctionreduction better than other parameters， which provides a basis for accurate clinical diagnosis and treatment.
