醫(yī)養(yǎng)結(jié)合機(jī)構(gòu)開展老年人群“六養(yǎng)”健康管理新模式的實(shí)踐探索

摘 要 本文梳理了老年人群健康管理的重要意義、面臨的機(jī)遇與挑戰(zhàn)及華東療養(yǎng)院探索開展老年人群“六養(yǎng)”健康管理模式的實(shí)踐情況，闡述了“六養(yǎng)”的主要服務(wù)內(nèi)容、實(shí)踐成效及未來展望等，為醫(yī)養(yǎng)結(jié)合背景下養(yǎng)老服務(wù)體系建設(shè)提供思路。

Practical exploration of the new health management mode of “six cares” for the elderly in medical and elderly care integration institutions

ABSTRACT This paper summarizes the important significance， opportunities faced and challenges of elderly health management， and exploration of development of the health management mode of “six cares” for the elderly in East China Sanatorium， and elaborates on the main contents of the services， the effectiveness of the practice， and the outlook of the future for “six cares”， so as to provide the ideas for the construction of the medical and elderly care service system in the context of integration of medical and elderly care integration.
