【Abstract】ObjectiveTo investigate the clinical effcacy of esmolol combined with extracorporeal membrane oxygenation （ECMO）in the treatment of adult refractory septic shock with respiratory failure.Methods Inthis study，1OO patients withrefractory septic shock with respiratory failure inour hospital were randomly divided into an observation group （n=50）andacontrol group （n=50）.Thecontrol group receivedECMO treatment， while the observation group received esmololon thebasisof ECMO treatment.Inthe course of the study，the vital signs，inflammatoryreaction indexandorgan function index of the two groupswererecordedin detail，and the prognostic indexes such as sequential organ failure score，incidence of adverse events and succesful weaning recovery rate were compared between the two groups.ResultsAfter treatment，compared with the control group， the patients in theobservation group hadsignificantlylower heartrate，cardiac output，higher meanarterial pressure （MAP），lower C-reactiveprotein，white bloodcell count，procalcitonin，higherarterialoxygen content， higheroxygenation index and lower respiratory index，andlower acutephysiology，chronic health scoreI and sequential organ failure score.The incidence of adverse events was lower（ P <0.05）.ConclusionEsmolol combined with ECMO hasgood clinical eficacy in the treatment of severe sepsis，whichcan significantly improve the vitalsigns of patients，reduce inflammatory reactions，protect organ function，and have feweradverse events.
